Can iron deficiency be a real problem if I am not yet anemic?

Yes. Many patients experience significant symptoms from iron depletion before hemoglobin falls below normal. The clinical relevance depends on the full picture — symptoms, ferritin level, rate of change, and other factors. A specialist evaluation can determine whether treatment is warranted.

When does IV iron make sense instead of oral iron?

IV iron is often considered when oral iron causes significant GI side effects, when the underlying cause of depletion is ongoing blood loss or malabsorption, when the anemia needs to correct more quickly, or when oral therapy has been tried and failed.

My ferritin is elevated. Does that mean I have iron overload?

Not necessarily. Ferritin is also an acute-phase reactant and rises with inflammation, liver disease, and metabolic conditions. True iron overload is defined by elevated transferrin saturation alongside elevated ferritin, often confirmed with genetic testing. Context is everything.

What is hereditary hemochromatosis and is it serious?

Hereditary hemochromatosis is a genetic condition that leads to excess iron absorption over time. When identified early and managed with therapeutic phlebotomy, outcomes are generally excellent. The degree of iron loading, associated organ risk, and mutation type all inform how actively it should be treated.

Does insurance cover iron specialist visits?

In most cases, yes. Visits for evaluation and management of iron deficiency anemia and iron overload are covered under standard medical insurance. IV iron infusion coverage varies by plan and indication. The office can assist with prior authorization when needed.

Dr. Kelly Casteel is board certified in hematology and medical oncology. She completed her fellowship training at MD Anderson Cancer Center, after which she served on faculty there. While at MD Anderson, she was recognized as a leader in training future oncologists and achieved patient satisfaction scores in the top 1% nationally.

Dr. Casteel has a special clinical interest in blood diseases including iron deficiency, anemia, hemochromatosis, blood clotting disorders, and platelet disorders. She maintains close relationships with local and national experts and brings that network to her patients in Longmont through Rocky Mountain Cancer Centers.
